Disabled Placard or License Plate
Any Kansas resident who has been certified by a Healing Arts Licensed professional may be issued a disabled placard or license plate.
How do I apply for a disabled placard or license plate?
- Fill out the following application: KDOR Certification of Disability for Disabled Parking Placard/Plate/Decal.
- Submit your application:
- By fax to 913-715-2510
- By E-mail
- By mail to Mission Motor Vehicle
How do I renew a permanently disabled placard?
Online:
- Renew your placard online through Ikan at https://ikan.ks.gov/. If you renew online, you do not need to complete or submit the KDOR Self-Certification of Continued Eligibility for Disabled Parking Privilege form.
By mail, email, fax or in person:
- If you are not renewing online, complete the KDOR Self Certification of Continued Eligibility for Disabled Parking Privilege form and submit it by one of the following methods:

- By email to dmv@jocogov.org
- By mail to Mission Motor Vehicle
