Johnson County Department of Health and Environment's Community Health Workers

The Johnson County Department of Health & Environment Community Health Worker (CHW) program is a free resource that helps clients learn to navigate applications for Kansas Medicaid, Presumptive Eligibility and SNAP, as well as connect to various community resources.

Clients who participate in the CHW program will meet regularly with a certified CHW to identify goals, work toward goal attainment and gain the knowledge and confidence to navigate resources and systems independently.

Community Health Workers are trusted members of the communities they serve who have a deep understanding of health systems, barriers to health and healthcare access and community resources.
