Open to children ages 5-11 years old from 8-11:30 a.m. A parent or guardian must be present.

Save time in line by completing this form (en español) and bringing it to the clinic for the child’s first or second dose. Parents/guardians should also bring their child’s COVID-19 vaccination record card for the second dose.
