Students entering preschool through 12th grade in August 2025 can get caught up on vaccinations at one of three walk-in immunization clinics in July and August 2025. 

Each clinic will provide all recommended childhood vaccines and school-required vaccines. Parents are encouraged to complete paperwork using this online form. A parent must accompany the student to the clinic. No appointment is needed. 

Students entering the 11th grade must be 16 years or older to receive the second (booster) dose of the meningococcal ACWY (meningitis) vaccine.

What to bring:

• Signed Consent Form (if not completed online): Immunization Consent Form or Formulario de Consentimiento Para las Vacunas
• Insurance card (if you have one)
• Student's vaccination record (if you have one)
• Letter from the school (if you have one)
