These forms are here to assist you with all of your document filing needs

  • Certificate of Release of Prohibited Covenant

  • Cover Sheet - This form is used for documents which do not comply with margin requirements.

  • Escrow Account Access Request Form - TTV shall establish an escrow account in Customer's name to be used by customer for payment of recording fees, copies, on-line subscription fees, and charges assessed by TTV.

  • Kansas Open Records Request - You have the right to inspect and obtain copies of public records that the Treasury Taxation and Vehicles maintains which are not otherwise exempt from disclosure by a specific law.  Your request for public records must be in writing.  You will be charged a fee ($.25 per page) for copies. 

  • Kansas Real Estate Sales Validation Questionnaire - (aka Certificate of Value) This SVQ form is required by the State Property Valuation Department(PVD). It must be completed with each deed that is recorded in the Register of Deeds Office unless the deed meets one of 16 exemptions listed on the back of the form. If your document is exempt, it must be clearly stated on the document being recorded.

  • Request to Combine Real Estate - This Form is used to combine real estate for tax billing purposes.
