How do I get the renewal for my disabled parking?

The form used to request a replacement placard may be found on the Treasurer website at: http://treasurer.jocogov.org/dmv/ dmv_forms.aspx.  Under Motor Vehicles Forms, click on the Self Certification of Continued Eligibility for Disabled Parking Privilege link.  That link will take you to the form that will allow you to renew the Placard.
