Case Management

The JCDS case management team is comprised of degreed, experienced professionals, who are dedicated to empowering individuals to lead enriched, interdependent, healthy, safe and choice-driven lives. We take pride in providing responsive customer service, arranging for an extensive variety of individualized services, and developing close working relationships with each person and their support network of family, employers, teachers, and service providers.

Case Managers provide support and assistance to the individual with the developmental disability, their guardians, and families to

  • Discover the person’s lifestyle preferences, strengths, skills, behavioral, health, safety, and resource needs.
  • Identify and access resources and opportunities needed to increase independence, productivity, inclusion and integration.
  • Develop a person centered plan to meet personal goals and support needs.
  • Evaluate the implementation of the person’s plan and their satisfaction with services and supports.
  • Monitor services for quality, responsive, and individualized supports.

JCDS provides targeted case management services to children and adults with a developmental disability and with all types of disabilities and levels of support needs. If you have JCDS Case management, you may choose to receive your other services by any service provider licensed with the State of Kansas and affiliated with the Johnson County CDDO. It is your choice! JCDS case managers also provide independent living counseling for individuals in the Medicaid WORK program, and a private pay option for individuals who are not eligible for Medicaid.

Day & Employment Services

JCDS promotes the values of “Employment First” with a focus on integrated, community-based employment for individuals with intellectual and other developmental disabilities to earn at or above the minimum wage as the first option. If competitive integrated employment is not an immediate goal or inaccessible in the short-term, JCDS offers a variety of on-site and community-based employment and community volunteer service options for individuals with all levels of support needs. For retired citizens, and for those impacted by significant health-related conditions, JCDS provides a variety of meaningful day activities.

Employment Services

For individuals who seek paid employment, JCDS assists job seekers to obtain and retain employment preferably as a direct employee of a local business. As a next best option for integrated community employment, JCDS enters into contractual agreements so disabled workers employed by JCDS work on the premises of local businesses. Such arrangements include small groups of JCDS employees who work alongside peer employees of the business, as well as small mobile work crews for jobs such as servicing vending machines.

For workers who have not secured employment in the community, JCDS provides a center-based workforce for businesses who desire to have their products assembled and packaged on premises other than their own. Most center-based employment is paid piece-rate work at a wage based on individual productivity.

Community Services

Some individuals, for whom employment is not a priority, prefer to make meaningful contributions to their community through volunteerism. Examples to where our folks have chosen to donate their time, energy and creativity are Meals on Wheels, Children’s Mercy Hospital, Hospice, Turnstyles, various Skilled Care Facilities, and Animal Haven.

Retirement Services

Many older adults with developmental and intellectual disabilities prefer to participate in day activities that are typical of those that many seniors enjoy during their retirement years. Some of the activities include movement classes, attending movies, memory retention activities, T-Bones baseball games, Rodeos, and music therapy. In addition, these participants are committed to giving back to their community like others. They have enjoyed contributing to Meals on Wheels, Wayside Waifs, Skilled Care Facilities, Adopt a Senior, and Harvesters, to name a few favorites.

Handbook

This handbook is provided to current and prospective recipients of supports and services from Johnson County Developmental Supports (JCDS) and their families and representatives. The handbook provides a summary description of the direct supports, and policies and procedures of JCDS (“Johnson County Government Rules and Processes”). After reviewing these services and exploring with your person-centered planning team the type of support you need, you may decide JCDS is your provider of choice.

Please keep this handbook while you are receiving services from JCDS. If you need help with understanding the handbook, your case manager or staff support person will read and explain it to you. We have tried to cover the basic information you need to be an active participant in your supports and services. If you have specific questions, please ask any JCDS staff member for help.

JCDS Services

Provider Services

JCDS is a Community Service Provider supporting adults with intellectual and other developmental disabilities who reside in the Johnson County Kansas community. JCDS Case Management Services, Employment & Day Services and Residential Services are licensed by the State of Kansas and for over three decades have maintained the highest level of international accreditation through CARF. Our driving focus is to assist our primary customers to achieve independence, productivity, integration and inclusion.

Day and Employment Services
JCDS promotes the values of “Employment First” where persons with intellectual and other developmental disabilities can achieve a greater level of prosperity as productive members of the workforce. We focus on integrated, community-based employment where workers earn at or above the minimum wage as their first option. Whether "home" means living in a house or an apartment, living alone or with others, JCDS provides just enough supports for each resident to live their own productive life as independently as possible regardless of how much or what kind of support is necessary for each resident.

Case Management Services
Attempting to navigate the service system for people with intellectual and developmental disabilities can sometimes feel like a maze. That’s where case management services can lend a hand! We take pride in providing responsive customer service, arranging for an extensive variety of individualized supports and services, and developing close working relationships with each person and their support network of family, employers, teachers, and service providers.

Residential Services

Whether "home" means living in a house or an apartment, living alone or with others, JCDS provides just enough supports for each resident to live their own productive life as independently as possible regardless of how much or what kind of support is necessary for each resident. Those individualized supports vary from one person to the next and may include comprehensive, on-site staff support 24 hours per day in their homes, or as little as a few hours a day for a couple of days per week. As is typical for everyone, each person’s support needs likely will change over time, either in need of less staff support or greater staff support. No matter the case, JCDS seeks to remain a constant partner as the person’s needs evolve over time, assisting them to achieve the lifestyle they desire in the mainstream of our Johnson County community.

While many participants in JCDS residential services obtain and prefer their own housing arrangements, many have relied on the affordable and accessible houses owned by the Friends of JCDS. The Friends of JCDS is their landlord, while JCDS Residential Services provides daily supports to the residents of those homes. It is a unique and successful partnership between the residents, their landlord, and their human service provider.
